About the episode

Dr Pradeep Yadav will present a TTVR case in which 3D ICE serves as the primary imaging modality, illustrating right-sided anatomy, catheter–imaging interaction and key intraprocedural decision points.

Overview

This "Nail It with Nadira" case-based webinar brings together an expert tricuspid heart team to discuss the evolving role of three-dimensional intracardiac echocardiography (3D ICE) in transcatheter tricuspid interventions. The session highlights 3D ICE in transcatheter edge-to-edge repair (T-TEER) and transcatheter tricuspid valve replacement (TTVR). Moderated by Dr Nadira Hamid, the faculty will review clinical applications of 3D ICE in tricuspid practice, with discussion of workflow optimisation, imaging approaches and procedural efficiency.

Dr Pradeep Yadav will present a TTVR case using 3D ICE as the primary imaging modality. Lynne Brown will outline key 3D ICE views, measurements and reconstructions that may support patient selection, leaflet interaction, device positioning and assessment of residual tricuspid regurgitation. Dr Martin Swaans will discuss emerging developments in tricuspid imaging, including fusion imaging in TTVR, advanced three-dimensional rendering and integration of artificial intelligence into clinical workflows.

The session will conclude with a panel discussion and live audience Q&A addressing practical challenges such as limited TEE windows, complex right-sided anatomy and hardware, large coaptation gaps and multidisciplinary team training considerations. Participants will gain experience-based insights and practical perspectives on how 3D ICE can be incorporated to support and scale tricuspid programmes in real-world clinical settings.

Key Learning Objectives

  • Critically appraise the evolution of 3D ICE in TTVR and define when it adds incremental value over TOE and other imaging strategies.
  • Identify and acquire the key 3D ICE views and reconstructions that drive confident case selection, leaflet grasping, coaptation assessment and immediate result optimisation in TTVR.
  • Deconstruct a complex TTVR procedure guided predominantly by 3D ICE, including pre procedural strategy, intraprocedural pivot points and post implant troubleshooting.
  • Evaluate emerging innovations in tricuspid imaging—hardware, software and workflow—that may alter access routes, anaesthesia strategies and lab staffing models.
  • Integrate tips, workflow hacks and team-based approaches from high volume centres to reduce procedure time, radiation and contrast while maintaining or improving outcomes.

Target Audience

  • Experienced structural and interventional cardiologists performing or preparing to perform TTVR
  • Advanced imaging cardiologists and echocardiographers leading structural heart imaging programs
  • Senior fellows in interventional/structural or multimodality imaging tracks
  • Cath lab physicians, advanced sonographers and structural heart coordinators in centres developing or scaling tricuspid programs
  • Heart team members seeking to refine their use of 3D ICE as a primary rather than backup imaging modality for tricuspid interventions
